First visit form

When you come in for your first appointment with a dentist, you will have to fill out a form giving us all the relevant information on your health. To speed things up, please fill out the form at home.

  • 1. First, read this document on what to expect before you visit.
  • 1. Click on this link to download the document (pdf).
  • 2. Print and fill out the form.
  • 3. Bring it with you at your first visit

Appointment cancellations must be made by telephone or e-mail at least 48 hours in advance or a service charge will apply.

Your dental insurance forms are free and are sent electronically (over the Internet). In most cases, the amount of your coverage is available immediately. You pay the total fees owed to the dentist at the moment of the appointment.

Insurance forms that cannot be sent electronically are completed free of charge and will be given to you in person. You will then have to send them to your insurance company and pay the total fees owed to the dentist at your appointment. Treatment plans and estimates for different treatments can be prepared for a 30$ fee. The 30$ fee will be waived upon completion of the treatments.

We would also like to mention that your insurance coverage varies according to the agreement signed between the insurance company and your employer. It is illegal to overcharge your insurance or not to claim the balance from you. It is therefore crucial that you find out what your coverage is and your maximum annual entitlements.
